CREST SAGE @ Sengkang

There are over 100 community mental health teams in the community islandwide.  These community mental health teams, such as Community Outreach Team (CREST), serve as your in-person First Stop for Mental Health.

We promote early identification of at-risk individuals and provide psychosocial intervention to support those with mental health need and their caregivers in the community.

We also work with other community partners to improve mental health and dementia literacy as well as conducting preventative activities to reduce the risk of mental health and dementia issues, such as cognitive stimulation activities.

The community mental health teams are developed under the the Community Mental Health Masterplan driven by the Agency for Integrated Care (AIC), together with the Ministry of Health (MOH), which enables persons living with mental health issues and dementia to seek early treatment nearer to their homes and ensure that they are well-supported in the community.
